TERMS & CONDITIONS
I understand that risks are involved in physical activities such as dance, tumbling, and fitness that may result in serious injuries or death. I hereby release United Dance Center LLC & United Preschool Center LLC and all staff members and officials from any claims of damages or injury suffered by the registered participant in connection with or by their association with United Dance Center LLC & United Preschool Center LLC. This includes my heirs who may act in our behalf.
I give my permission to use photos of my student / participant online (for the website, Facebook, etc) or in an advertisement.
Tuition:
I understand that tuition is determined on an annual basis and is not pro-rated monthly. Some months have more classes than others due to holidays, extra practices, longer/ shorter months, etc.
I understand that tuition is not discounted if class is missed and make up classes are not offered.
I understand that tuition is due on the 1st of each month. The $10.00 late fee will be assessed for all payments made after the seventh of the month.
I understand that all United accounts are set up on Auto-Pay. Auto-Payments are withdrawn on the 1st of each month (September - May).
I understand that if I choose to pay with a debit or credit card through the online portal, a 3.5% card fee is charged per transaction. I understand there is NO FEE for ACH transactions.
I understand that no refunds are given for any reason. I understand that tuition is not lowered if classes are unattended no matter the reason. I also understand that if tuition is more than 45 days late the student will not be able to participate in class.
I understand that all balances on my account must be paid in full before receiving any costumes or performing. I understand that if my account goes 90 days past due, it will be sent to collections.
Registration Fees for Dance Recreational Classes: I understand that a registration fee of $35 for one dancer is due within one week of registration or a $55 fee is due for a family. No registration fee is due for summer camps.
Costume Fees for Recreational Dance Classes: I understand that all dancers will participate in the winter and spring recital. The costume fees are $60.00/student per dance class taken. The $60.00 Winter Recital Costume Fee will be due by September 1st. The Spring Recital Costume fee will due by December 1st.
Competition Dancers: Please refer to the competition team packet for team pricing.
Class Attendance:
Consistent class attendance is extremely important. One student’s absence effects the entire class of students, therefore all students are expected to come to class each week on time. Students are expected to commit to the class they are registered in for the entire school year. If your student must miss class we ask that you let us know ahead of time. We do not have make up classes for students and tuition will not be discounted for missed classes.
Withdrawal Policy:
I understand that If a student/participant needs to withdrawal from a class, his or her parent or guardian must submit a withdrawal note in writing to United Dance Center or United Preschool Center. Monthly tuition is posted on the 1st of each month. Parents must submit a withdrawal request prior to the 1st of the upcoming month. Verbal withdrawal notices or text messages will not be accepted. Refunds are not given if a student withdrawals from a class.
